I already stop paying my monthly contribution in Pag ibig for almost 5 years as OFW & completely withdraw?

Now I want to continue my membership do I have to use my old ID number or i have to registered for a new membership . I want to avail loan for LOT purchase..Do i have to fill up another application form?Please advise what i have to do. thank you and rgds...
